New Patients

Your First Consultation

Prior to your scheduled appointment, please remember to fill out the "New Patient Application Form" below.

During your first visit to Barrington Chiropractic, an examination of your spine and/or other joints of the body, for which you are seeking help, will be performed; including jaw joints, knees, wrists, and shoulders.

 

If you have concerns about headaches, dizziness, balance, or other problems relating to thinking or perception, a Functional Neurology examination may be performed with focus on movement of the eyes and body.

 

A personalized treatment plan will be developed to address the problems found in the examination.

New Patient Application Form

After making an appointment, in order to expedite your care, please download the New Patient Packet below, complete the form with your information, and bring it with you to your scheduled appointment.

 

In addition, you will also need to bring:

​

  1. A government-issued photo ID (current driver's license or state ID card)

  2. Any insurance cards (including a Medicare card) that you want to have billed - See Accepted Insurance​

Will I Need X-Rays?

X-rays are typically not required to start treatment. If x-rays or other medical imaging tests are necessary, we will refer you to an independent radiology laboratory for this service.
